The Transitional and Independent Living Program |

| Project Description |
Transitional and Independent Living Programs provide a continuum of apartment based housing and support services to older youth, helping them to learn the skills necessary for healthy independence within the context of a supportive team of professionals and an individualized plan tailored to their unique needs and strengths. The Aging Out Program provides a variety of training and support services, especially for youth preparing the leave the foster care system, aimed at improving educational attainment, independent living skills, and self-advocacy. |

| Eligibility Criteria |
Eligible youth include St. Louis at-risk youth (both male and female), ages 15-19, such as those who are preparing to age out of foster care, homeless youth who are not ready to live on their own without support, youth without parents, and disconnected youth who have no adult supervision. |

The Parenting Transitional Living Program |

| Project Description |
The Parenting Transitional Living Program (PTLP) supports homeless teen parents in a scattered-site, transitional living setting, providing housing, case management, food, basic life skills training, 24-hour on-call crisis intervention and basic necessities while training them to be effective parents through in-home and group sessions using the evidence-based, Born to Learn curriculum. In community settings, The agency will support at-risk teen parents in their placements throughout St. Louis County by providing in-home training and community-based group sessions using the evidence-based Born to Learn curriculum. |

The Family Focus Program |

| Project Description |
The Family Focus Program will provide in-home family and individual therapy, case management, resource linkage, respite care, psychiatry, 24-hour on-call clinical support, planned and emergency crisis intervention, emergency funds, and related support within the context of an individualized, family-based plan of care aimed at preventing a return to residential treatment or other more restrictive care setting. |

| Project Description |
The Youth Emergency Services (YES) Program is a crisis shelter that serves homeless, runaway, abused, and neglected youth, as well as youth at risk of abuse or neglect. YES provides a broad range of services, including food, safe shelter with 24-hour supervision, clothing, case management, resource linkage, therapy (individual, group, and family), and in-home aftercare services. |

| Eligibility Criteria |
Eligible clients include at-risk youth (both male and female) ages 11-19 who are in crisis, including youth who are homeless, those who have been victims of abuse or neglect, those experiencing family crisis or trauma, and those who are disconnected from family or adult supports. Services are provided irrespective of custodial status. |

Epworth’s Street Outreach Program provides center-based and street-based outreach to homeless, runaway, disconnected and at-risk youth. Project Description
|
Epworth’s Therapeutic School Program provides education, therapy, rehabilitation, and/or transition services to students attending its Purchase of Service special education program or its Webster Groves School District satellite classroom, both located on Epworth’s Webster Groves campus.

Project Description
|
The School Links program provides in-home counseling, skill-building, and parenting support to families of school-aged children who are experiencing challenges related to their child’s behavior and functioning at home and school. Our goal is to support the child, family and school in order to build the child’s and families’ skills and to improve their overall functioning.
Project First Steps provides in-home counseling, skill-building and parenting support to families who are experiencing challenges related to children’s behavior, family stress and conflict, and issues related to mental illness or disabilities. Our goal is to improve parenting skills, reduce parental stress, and improve the functioning of all family members.

Eligibility Criteria

School Links serves families of children ages 5 to 13 who reside in one of our thirteen partner districts in St. Louis County (Affton, Bayless, Brentwood, Ferguson-Florissant, Hancock Place, Lindbergh, Maplewood/Richmond Heights, Mehlville, Normandy, Ritenour, Riverview Gardens, University City, and Webster Groves). Families, school staff or community agencies can make referrals by calling 314.644.5055 x 120. |

Group Parenting Education and Support |

| Project Description |
Family Support Network will provide Group Parenting Education and Support services to the youth consumers of the St. Louis County Youth Programs at the Spanish Lake site. Using the Active Parenting curriculum, young parents will participate in four sessions of group parenting education and support with the goal of improving parental knowledge and skills, reducing parental stress, and improving outcomes for children. Services will be provided at the Doors to Success program and/or the Youth Resource Center in Spanish Lake.

This program will serve young parents who live in St. Louis County, are parenting children ages 13 and under, and are customers of St. Louis County Youth Programs in Spanish Lake. |
