Bridges to Success (IOP) is a collaborative afterschool/ intensive outpatient program which combines intensive substance abuse education and outpatient treatment services with ancillary services that include family therapy, mentoring, psychiatry, job and life skills training, academic support, credit recovery, leadership development, parent group and case management. |

Project Description
|
Bridges to Success ACAT (Assertive Community Addiction Treatment) is a community based model of substance abuse treatment for young people 13-19 being released from St. Louis County Justice Center, Juvenile Detention, the Division of Youth Services, residential treatment facilities, and/or are currently in a day-treatment/IOP program. ACAT is a composed a multidisciplinary team which provides traditional and non-traditional substance abuse treatment services primarily out of the office that are convenient for the family/adolescent. ACAT is assertive, long-term, substance abuse treatment, that emphasis educational/vocational supports, provides psycho-educational services, family therapy, places emphasis on community integration, and coordinates health care. ACAT’s goal is to keep adolescents in recovery and out of hospitals, residential programs, and institutions.

The program is geared toward adolescents between the ages of 13 and 19 who have been identified as at-risk for illicit drug and/or alcohol abuse/dependence, have experienced recurrent episodes of care in traditional residential and/or outpatient settings, and who have been referred for treatment by the St. Louis County Justice Center, Family Court, the Division of Youth Services, residential treatment facilities, and/or are currently in a day-treatment/IOP program. |

Bridges to Success (BtS) In-Custody Treatment |

Project Description
|
Bridges to Success In-Custody Treatment will provide In-Custody Substance Abuse Treatment and Re-Entry services for adolescents aged 17-19 who are moving through the St. Louis County Justice Center. In collaboration with the Justice Center and Queen of Peace, in custody treatment and re-entry services will be provided to offenders who are housed in the St. Louis County Justice Center. The In-Custody Treatment component of this program will utilize a traditional Intensive Outpatient Treatment model combined with intense case management and re-entry planning. An adapted version of this model will be used and crafted to fit the needs of the population based on length of stay and individual needs.

Project Description
|
Bridges to Success Day-Treatment is a day-treatment program which provides intensive substance abuse treatment/education, in a 6 hour per day, 5 days per week setting, as well as ancillary services that include family therapy, psychiatry, mentoring, job and life skills training, academic support, credit recovery, leadership development, parent group and case management. Clients will be assessed using the GAIN assessment and upon completion of the assessment a comprehensive treatment plan and re-entry plan will be collaboratively developed by the treatment team.

The program is geared toward adolescents between the ages of 13 and 19 who have been identified as at-risk for illicit drug and/or alcohol abuse/dependence, who are in need of a more intense level of care than traditional outpatient, and who have been referred for treatment from the juvenile court, agencies, DJS, school districts, families, and/or self-referral.
